Click on "Edit" on the menu bar at the top of your screen and then click on "Preferences." A box will open on your screen (you can move the box so that you can still read this message).
2. Click on "Advanced" in the left column.
3. Check the box next to "JavaScript Enabled."
4. Click on the "OK" button at the bottom. The box will close.
5. Click on the "Reload" button at the top of your browser. If you no longer see this message, JavaScript is now enabled and you can use this form.

Internet Explorer1. Click on "Tools" on the menu bar at the top of your screen and then click on "Internet Options." A box will open on your screen (you can move the box so that you can still read this message).
2. Click on the "Security" tab at the top of the box.
3. Click on the button near the bottom that says "Custom Level."
4. Scroll down until your see the heading "Scripting." Under "Active Scripting," click on the circle/radio button that says "Enable."
5. Click on the "OK" button at the bottom and then click on "Yes" when you are asked "Are you sure you want to change the security settings for this zone?"
6. Click "OK" at the bottom of the box and the box will close.
7. Click on the "Refresh" button at the top of your browser. If you no longer see this message, JavaScript is now enabled and you can use this form.
